For Extension of API 1104 Pipeline Welder Certificate
Required Documents for Extension
1. Copy of ID Document
2. Submit your application by clicking the button “Re-certification" at https://oim.ugetam.istanbul/basvuru. And fill in and sign the Application Documents provided there.
3. Film and report of the welding you performed in the recent 6 months, to be certified by Gas distribution company or consulting company
4. If you have sent signed copies of documents required at 1), 2) and 3), every 6 months and obtained approval, then you are not required to re-submit them.
5. Then-current re-certification fee.
These documents must be delivered to the address “UGETAM AŞ BELGELENDİRME ŞEFLİĞİ ÇAMLIK MAHALLESİ YAHYA KEMAL BEYATLI CAD NO 1 KURTKÖY PENDİK İSTANBUL" until expiry date of the certificate.
