Extension of 10UY0003-4 Flue Inspector Level 4 Certificate
You must submit your application no later than 10 days prior to expiry of your certificate since approval process, and notification to PQA process cannot be handled during weekends.
First thing to do for application is to click here and then enter your password, if you know your password, or if you do not know your password, click here to get one. Then enter the password sent to you to log in, and then click the “Re-certification" button to submit your application.
Required Documents. 1. Signed certificate request form and agreement. You can click here to log in with your password, and download them from my applications menu.
2. Records of applicants which shows that he/she worked in the relevant area for a total of at least two years or, uninterruptedly in the recent 6 months, within the validity period of his/her certificate (job service details, reference letter, contracts, invoice, portfolio, etc.),
3. Letter of recommendation/service which indicates that he/she has worked as “FLUE INSPECTOR" in the recent 6 months “…… (name) ……. (surname) Turkish ID ……….. has been working as a Flue Inspector for ……….. months/years. “
4. Copy of ID Document
After submission of your application, do not forget to upload the documents that are required to be uploaded to the system. Your documentation will be checked to verify if they are suitable, the result of which will be notified you via SMS.
After approval of your application, the payment menu will open, and you can make your payment online here, or, you can upload the bank receipt you received which contains your name/surname and Turkish ID for bank transfer you made to the bank account.
These documents must be delivered to the address “UGETAM AŞ BELGELENDİRME ŞEFLİĞİ ÇAMLIK MAHALLESİ YAHYA KEMAL BEYATLI CAD NO 1 KURTKÖY PENDİK İSTANBUL" until expiry date of the certificate, and postage or cargo fees will be paid by you.
